Looks like Nautilus-Elementary with breadcrumbs enabled. To change up the order of the nav-bar, you can right-click on it->"customize toolbar"

I mean the navigaton buttons left and right <- ->. The seem to be together, like in Mac OS X finder.

The navigation buttons were done using a trick I learned from seahorsepip. It's just a toolbar background with the navigation buttons embedded in.

I think that's probably custom icons in the gtk theme, but not sure.
